An inquest into the death of Sydney student David Iredale has recommended a full review of ‘000’ services – the equivalent of the UK’s 999 – after finding that call handlers were preoccupied with getting a formal address before they would help. 17-year-old David Iredale became lost during a hike through the bush in December 2006. He ran out of water and called the emergency services from his mobile phone; the coroner said the call takers had a "relentless focus" on obtaining an address and should have overridden the system.
